General practitioner care for Ukrainian refugees in Breda
Thuisdok has made agreements in collaboration with the GP care groups (HZG and Het Huisartsenteam) and the municipality of Breda to contribute to general practitioner care for Ukrainian refugees who are staying in the Koepel or in the vicinity of Breda.
What does Thuisdok do?
- Thuisdok registers the refugees in their HIS, so that dossiers are created
- The registration form is provided in Ukrainian language for easy registration
- Thuisdok can be reached by phone (callback request) on regular weekdays from 8.00-17.00 for (care) questions
- Thuisdok offers (video) consultation hours and the option of e-consultations, this option is still available in Dutch and English.
- If a physical assessment is required, Thuisdok will arrange for the scheduling of the appointment and access to the HIS for the assessing GP at one of the Thuisdok's practice locations.

How can you receive GP care
If there is a need for care from a Ukrainian refugee and he/she is not yet registered with the Thuisdock:
- The patient can be registered at Thuisdok using the registration form below. The request is to state a short description of the request for help.
- A callback request can be made by calling us and choosing option number 3. Provide a name and phone number and the assistant will contact you.
After the registration of the patient, a triage is performed to assess the seriousness of the complaint(s). Then we look at how we can best help the patient; remotely or at one of the Thuisdok practice locations.
If there is a need for care for a patient who is already registered with Thuisdok:
A callback request can be made by calling our telephone number and choosing option number 3.
